About this template
The first session goes better when it is not spent on questions. This consultation collects training history, current activity level, injuries, goals, and the days somebody can realistically train.
Injury history is the field to keep required. A programme written without it gets rewritten after the first session, and the client remembers which one they paid for.
Trainers working with older clients often add a medical clearance question above the goals section.
