About this template
Donation pages lose people at the point where the amount is not obvious. This form offers set amounts alongside a free entry, asks whether the gift is one time or recurring, and captures what you need to claim tax relief.
The dedication field is small and matters. Gifts made in memory of somebody arrive with a name attached, and that name belongs in the letter you send back.
Connect a payment processor to take the money at submission. Without one the form records a pledge and you collect separately.
