About this template

Volunteer coordination fails on availability, not enthusiasm. This form asks which roles somebody is interested in, which days and shifts they can cover, what experience they bring, and who to call in an emergency.

Availability collected as checkboxes turns into a rota you can build from. A sentence saying “weekends mostly” does not, and you will be the one translating it at eleven at night.

Add a background check consent question for roles that involve children or vulnerable adults.
